  • 2025 Open Enrollment

    Open Enrollment for an effective date of January 1, 2025 will begin November 1, 2024 thru November 30, 2024. Below you will find health plan coverage overviews as well as enrollment information and required forms.

    In collaboration with Blue Cross Blue Shield (BCBS), AFA is offering three medical plan options for 2025, tailored to meet a range of coverage and budget needs:

    Access 500: Comprehensive BCBS coverage.

    Premier Plan: BCBS coverage with secondary support through AmFirst/Alliance.

    Value Plan: Affordable BCBS coverage with essential benefits.

  • What is seconday insurance?

    Secondary insurance through Alliance pays most deductibles and out-of-pocket expenses up to the policy limit after your primary medical plan has paid its covered expenses. For the AFA Premier Plan, your secondary coverage pays a total of $7,500 toward your deductible and out-of-pocket maximum, meaning you only have to pay $500 of your own money. To receive full benefits, be sure to give your doctor both your primary insurance card and your secondary insurance card.

  • Who is Eligible for the AFA Group Health Plan?

    All current AFA members who have at least one common law employee are eligible for the AFA Health Plan. Sole proprietors must have at least one common law employee to participate in the plan. Please contact Harmon Dennis Bradshaw at 334-517-1846 with questions.

    Below we have linked below all of the required forms necessary for your company to enroll in the AFA Health Care Plan. Once completed please return all completed forms to:

    Required for those choosing the Premiere Plan which includes secondary coverage through AmFirst/Alliance.

           Secondary Coverage Application

    Required for Those Electing Dental and Vision or Just Dental Coverage

           Canopy Insurance Application

    Upon the return of your re-enrollment packet you will be contacted by Harmon Dennis to finalize your company’s re-enrollment details.

  • Can I elect Dental, with out participating in the Medical Plan? Also, can I celect Vision as a Solitary Benefit?

    Yes. Both Dental and Medical can be elected separate of each other, or together.

    As for Vision, no. Vision can only be elected if Medical and/or Dental is elected. It cannot be elected by itself.

  • What to do to add/remove employees from the benefit? How is the benefit offered to new hires?

    Once a member company is enrolled and established in the EASE management system, the member company will be responsible for submitting adds/terms/changes of any employee who elected the plan or terminated each month. This will ensure each group is billed the appropriate amount each month. Changes can be made in the EASE portal. Need help or have questions? Reach out to our team any time:
